INTRODUCTION TO SINGAPORE PROGRAMME FOR INTEGRATED CARE FOR THE ELDERLY AND INTEGRATED HOME AND DAY CARE PACKAGES

Frail elderly who need more attention at home can get the care they need, if going to a nursing home is not on the cards. Seniors who need nursing and medical care and therapy attend a centre that provides this programme. They get to take part in recreational and social activities also. Transport and meals are provided.

SERVICES PROVIDED

A team of professionals comprising nurses, therapists, social workers and doctors will work out a care plan for the senior. Services may include:

HOW TO APPLY


Ask a medical social worker or your care coordinator for a referral. They can help you find out if the senior is eligible for this programme​.

A case manager from the centre offering this programme will decide if your senior is suitable for the programme too. The programme costs between $1,600 and $2,200 a month, excluding transport services. 

If you​ need help paying for the service, please speak to your service provider or the Medical Social Worker.
